Advertising Form  If you would like to place an ad to appear in our Concert Programs, please fill out this form and mail it with your payment to Robyn Anderson.




Barnes & Noble Book Fair Voucher  On selected dates you can present this voucher at the Barnes & Noble register when you make your purchase, and a percentage of the net sale will be donated to our organization!  Contacts: TBD.



Expense Voucher  If you are a Booster Officer or Committee Chairperson and have made purchases related to your job, please fill out this form  and give it to Teresa Lee (Treasurer) along with your receipt.  [NOTE:  All expenses in excess of $100, whether included in the budget or not, require approval from the Board prior to incurring the expense.



Franco's Order Form  Use this form to place your order for Italian dinners.  See the fundraising page for details.




Silent Auction Forms  All Choral Booster families are encouraged to make at least one donation by requesting contributions of goods or services from area businesses.

To thank our silent auction contributors formally, please download a copy of the 2008 Silent Auction Thank You letter and update it with the appropriate information.




Do you know of an individual or business that might consider donating an item?  Print off a copy of the Silent Auction Letter (inserting your name and address) to mail or deliver to them.  The letter gives information on the purpose of the auction, instructions on how to make a donation, and Silent Auction Committee contact information.




Enclose with the Silent Auction Letter 2 copies of the Silent Auction Agreement.  This provides a way for the donating businesses/individuals to provide their contact information and give a description of the item to be donated along with an estimate of its value.  One copy is sent to Nirmal Bajekal (Silent Auction Committee), and the other copy is for their own records.

Before you approach an individual or business with a request to make a donation, please check the Donation Contacts Made document.  It contains a record of contacts made and what the status is regarding a donation.  This will help us to avoid making several requests of any business.  We will update it regularly.  If you make any contacts that you'd like to report, please email Nirmal Bajekal  She would also be happy to answer any other questions you might have.
